These are forms required by Licensing and must be completed and returned on or before first date of starting with BASE49.  These can be emailed back to Base49@CO01900838.schoolwires.net or printed and returned to your BASE49 staff members at your school.  If not received, suspension and dis-enrollment of BASE49 services will occur.  Please contact BASE49 office at 719-495-1182 with questions or concerns.
